Transcripts & Institution Evaluations

Transcripts

Transcripts contain all academic information about a student, including classes, grades, class rank (pre-doctoral students only), GPA and degree(s).  Graduating students, please note that your degree and graduation date will not appear on your transcript until all final grades have been recorded and the appropriate/official graduation date has passed.  Transcripts include the signature of the School of Dental Medicine Registrar and the Boston University seal.

Requests are normally processed within three business days.  There is no charge for this service.

How to Request a Transcript

Current Students
Current students must request transcripts online via the SDM Portal. Please use the following links.

On Campus: log-on to the portal (Kerberos login/password required).

Off Campus: log-on to VPN before going to the portal (Kerberos login/password required for both links).

Alumni & Former Students
Alumni must submit requests in writing, either by submitting a completed and signed Transcript Request Form.

Please email your completed transcript request form as an attached file to:

If you are unable to use the transcript request form, you can write a letter. All letter requests should contain the following information:

Full name, including any former names
Boston University I.D. number
Date of Birth
Dates of attendance
Degree(s) awarded, if any
Daytime phone number
Email Address
Number of copies
Complete address(es) of transcript destination(s)
A signature is required

Delivery Options

Transcripts may be picked-up in person or mailed. Note that in an effort to promote social distancing, we strongly encourage that transcripts be mailed to their recipients throughout the pandemic.

Appointments are required for pick-up, and visitors must adhere to all University COVID-19 guidelines.

Institution Evaluation Form

Students and alumni may request a letter of recommendation from the Dean of GSDM (i.e., an Institution Evaluation Form) for residency applications through the PASS or for non-PASS purposes as well.

Request Institution Evaluation

Students and alumni may also request letters of recommendation from the Associate Dean of Students, including personal letters of recommendation and the Professional Evaluation Form via the Postdoctoral Application Support Service (PASS). For more information on those letters, click here.
