Family Medicine Center

The Family Medicine Center, a Level-3 NCQA certified Patient-Centered Medical Home, provides comprehensive care for the whole family. With an integrated multidisciplinary care team, meets patients’ needs under one roof.

Geriatrics Program/ Nusing Home Group

The Geriatrics Program provides teaching and clinical care at Boston Medical Center, affiliated Community Health Centers and Boston area nursing homes.  Medical students participate in geriatric workshops and small group discussions on key aspects of care to the elderly. They visit an elderly patient on a home visit and perform a geriatric assessment.  Residents participate in geriatrics case conferences and seminar series.  They provide clinical care on home visits, the palliative care service, the East Boston PACE program, and hospice.  Residents are assigned longitudinal nursing home patients and visit them monthly starting in the second year.

The Family Medicine Nursing Home Group provides long term care, and care to patients in acute rehabilitation and skilled nursing settings in 5 nursing homes in close proximity to affiliated health centers.  The faculty team consists of nurse practitioners and family physicians.  The innovative model ensures close communication with the inpatient and outpatient consultative services thorough the integrated electronic medical record.   Our patients are admitted to the inpatient Family Medicine Service at BMC when they require hospitalization.   On discharge home to the community, we communicate directly with the primary care providers at the community health center. Our physician faculty and residents provide continuity to patients in the hospital, the nursing home and the community health center.

Ryan Center for Sports Medicine and Rehabilitation

A gift from Robert L. and Sharon G. Ryan, the state-of-the-art Ryan Center is the first full spectrum Sports Medicine practice in Boston, providing exceptional multidisciplinary service to clients who reap the benefits of a full continuum of care operating in one building.  The Ryan Center opened in 2006 and was renovated in 2012; the facilities include a SwimEx pool for aquatic therapy and more than 10,000 square feet of newly renovated space.

Physician services, offered in partnership with Boston Medical Center’s Department of Family Medicine, include musculoskeletal ultrasounds, on-site X-rays, exercise treadmill tests, and a concussion management clinic.  The Boston University Physical Therapy Center offers physical therapy services including aquatic therapy, musculoskeletal spine care, post-surgical rehabilitation, and therapy for sports and orthopedic injuries. The Boston University Sargent Choice Center provides nutrition services such as comprehensive dietary analysis, personalized food plans, and nutritional counseling.

Providers serve as team physicians at Boston College, Boston University, and Boston public schools.  Additionally, Ryan Center healthcare professionals lecture and conduct research at the Sargent College of Health and Rehabilitation.
