Commonly Used Forms
This page provides a listing of commonly used forms for researchers who know what they’re looking to do. If you are new to BU, to industry collaboration, or to any of the agreements or forms below, please reach out to BU Industry Engagement and we can help you get started.
- Incoming Material Transfer Agreement: A written agreement from the provider of the material to be received by BU.
- Incoming Addgene MTA Request Form: A written agreement from Addgene to be received by BU.
- Outgoing Material Transfer Agreement: A written agreement used by BU when sending materials outside BU.
- Confidentiality Disclosure Agreement: A written agreement to be used when disclosing or receiving proprietary or non-public information with an outside organization.
- Data Use Agreement: A written agreement to be used when sending and/or receiving data.
- Unfunded Collaboration Research Agreement: A written agreement to be used when establishing an unfunded collaboration with non-BU parties.
- Proposal Summary Form: A form for funded research projects, such as sponsored research or clinical trials.
- Non-federal Budget Template: A written template form to be used for industry-funded projects.
- Fee-for-service Summary Form: A written form to be used when submitting a fee-for-service proposal. This form must be signed by your department chair or center director and dean for any fee-for-service projects funded by an outside organization.
- IP Intake Form: A written form used to ascertain the likelihood of new protectible intellectual property being generated in the context of a particular project and the proper disposition of such new intellectual property. If requested by Industry Engagement, please fill out this form and return it to your Industry Engagement representative.
