About the speaker:
Dr. Susan Bergman has been a Physiatrist at Boston’s Community Medical Group (soon to be known as Commonwealth Community Care) since 1993. She is an Attending Physician at both Boston Medical Center (BMC) and Metro West Medical Center, and has been in private practice since 2003. Dr. Bergman worked as a staff physiatrist on the SCI unit at the New England Regional Spinal Cord Injury Center at BMC from 1985 to 2003, and has served as the Clinical Director of the SCI Center. She is also a consulting Physiatrist to nursing facilities in the area and has consulted to a number of area facilities, including Spaulding Rehabilitation Hospital (1986-1987). Dr. Bergman works with the Framingham Accountable Care Organization (ACO) and the New England Quality Care Alliance to expand the availability of comprehensive affordable coordinated care to patients and communities. She received her MD from the Boston University School of Medicine in 1982 and is Board Certified in Spinal Cord Injury Medicine. Dr. Bergman is an Associate Clinical Professor of PM&R at Boston University School of Medicine.
About Boston’s Community Medical Group (BCMG):
As one of the first groups in the nation to provide community-based primary care to adults with complex physical disabilities, BCMG offers a multi-disciplinary team approach to care that emphasizes preventive health care strategies, management of chronic conditions, as well as prompt responsiveness to new problems. It provides care that is individualized to meet patients’ needs and utilizes home visits to help overcome access barriers. BCMG enhances people’s ability to live independently in the community of their choice by providing care wherever it is needed whether the home, office, worksite, or hospital 24 hours a day, seven days a week.
About this month’s topic:
During this lecture, Dr. Bergman will give an overview of the development of SCI care in the United States, which has led to BCMG’s community- based model for primary care for individuals living with complex disabilities. Dr. Bergman will discuss the pros and cons of this primary care model. She’ll offer important information about how consumers can take charge of their health, work with their PCPs, and advocate for what they need in a primary care setting.
