Center for English Language & Orientation Programs (CELOP)

All incoming BU students must provide certain health history information before arriving on campus. Massachusetts law requires that college students have certain immunizations before coming to school. Failing to submit your health history information by the start of school may jeopardize your enrollment status.

1. Download and Complete Immunization Requirements Form

Please download and complete the CELOP Immunization Requirements Form.

Important reminders:

1. Forms must be filled out in English.
2. Forms should be completed and signed by a licensed medical provider (not your parents).
3. If you are unable to receive certain immunizations in your home country prior to arriving to the U.S. and your form is incomplete, this is not a problem! You will be able to receive them in Boston when you arrive and we will work with you scheduling an appointment.
4. If you are under the age of 18, your parent/guardian must sign the Consents and Acknowledgments section of the form.

2. Upload and Submit Immunization Requirements Form

1. Take a picture of your completed forms or scan them to your computer. There should be one picture for each page that has been filled out. If you have questions, please contact CELOP Admissions (celop@bu.edu)
2. Log into Patient Connect using your Kerberos account issued by the university.
3. Click the Medical Clearances option on the left menu bar and then click the Update button to the right of Immunization Record.
4. Click the Upload button and follow the instructions to upload your Immunization Form.
   
   1. Click Save once completed.

3. Enter your Immunization Requirements Form dates

1. Once the Immunization Requirements Form has been uploaded, log the immunization record dates for each immunization compliance requirement by clicking the Update buttons to enter your immunization record online using your completed Immunization Requirements Form.
   
2. Review and click Done when you are satisfied with each immunization record entry.
   

Unable to Submit Online?

We encourage you to upload the forms in Patient Connect  because it will update your compliance status much faster than standard mail. However, if you cannot complete the steps above because of technological restrictions, please send all completed paper forms to:

We cannot accept health information received via email.

All CELOP students are automatically enrolled in the Boston University Student BASIC Plan. If you are enrolled in BU’s Student Health Insurance Plan (SHIP), all immunizations are covered only if received at BU Student Health Services. Students not enrolled in BU’s Student Health Insurance Plan (SHIP) can still receive immunizations at BU Student Health Services, but will be charged on their Student Account. More information about the Student Health Insurance Plan (SHIP).

Massachusetts state law requires all full and part-time students to provide proof of immunizations. You must see a doctor in your home country and have your doctor complete the Immunization and Physical Form. Then, you must submit the form to us before arriving at CELOP by uploading the completed form online. The Immunization and Physical Form will be sent to you after you apply and are accepted to CELOP with instructions.
