Request Medical Records

Students are entitled to copies of their medical records and can access them using a written authorization. Medical records are kept for 20 years after the student’s last visit or treatment at SHS.

How to Request Your Records

Current and former students can request their medical records online.

For medical or mental health records:

• Complete the Authorization for Medical Release Form.
• Within 3 weeks, you will receive copies of your records. You can choose to have them sent to you via secure email (DataMotion) or have them faxed to a specified location (such as a doctor’s office).

For immunization records:

• Log onto Patient Connect
• Under the “Immunizations” tab, your record is available to view and print
