The SHS Patient Services group is dedicated to maintaining your medical records and keeping your health information private and secure in accordance with federal privacy and state regulations. Medical records are kept for 20 years after the patient’s last visit or final treatment. Patients have the right to review and receive copies of their medical records.
Immunization Records
Good news! If you are a current or former student, your immunization records are available to view and print via Patient Connect within the “Immunizations” tab.
Medical Records
Under federal and state law, a patient has a right to request a copy of their medical records. However, a completed written authorization is required prior to processing the request. For your convenience, SHS has made available a Medical Records Release Form that may be downloaded for your personal use, signed, and uploaded online via Patient Connect. To help us process your request, please follow carefully the instructions below to ensure that the authorization is accurate and complete.

When to expect your records?
Once we receive the form, your request can take up to 3 weeks for processing.
