The cause of this incident was determined to be a change that Microsoft implemented.

Description of Impact
Office 365 clients are receiving an error when trying to log into Outlook on the web.
Current Status
Microsoft found the problem and are in process of rolling back the change that caused it: Current status: Additional investigation determined that a recent change contained a code regression that resulted in authentication failures within the Outlook on the web. We’ve disabled the deployment and are currently in the process of rolling the affected change back to mitigate impact.
