Students must live in program housing. It is critical that this form is filled out by the deadline listed on your pre-departure page. Failure to do so may jeopardize your housing assignment. Keep in mind that your placement will be made based on the information you provide on this questionnaire. Please note that we cannot guarantee that we will be able to accommodate your preferences. However, if you have any special requests of which the Office and Residence Hall Manager should be aware, please be sure to note those on your form.
